Hiponatremia Grave Revelando Insuficiência Isolada de ACTH em Usuário Crônico de Álcool: Um Desafio Diagnóstico

Resumo

A insuficiência isolada de hormônio adrenocorticotrópico (IAD) secundária ao uso crônico de álcool é uma condição extremamente rara, frequentemente subdiagnosticada devido às manifestações clínicas inespecíficas que se sobrepõem às complicações do alcoolismo. Um paciente brasileiro do sexo masculino, de 42 anos, com histórico de etilismo crônico, foi internado por hematêmese e hiponatremia grave, refratária à terapia convencional. A avaliação bioquímica revelou hipocortisolismo acentuado, com níveis inapropriadamente baixos de hormônio adrenocorticotrópico (ACTH) e função hipofisária e exames de imagem normais. Uma investigação abrangente excluiu outras causas potenciais de hiponatremia. Foi então levantada a hipótese diagnóstica de IAD. A terapia de reposição com hidrocortisona resultou em rápida melhora clínica e na normalização dos níveis de sódio. Este caso destaca a importância de considerar a IAD em pacientes alcoólatras crônicos com hiponatremia inexplicável. O diagnóstico precoce e a reposição de corticosteroides são essenciais para prevenir complicações potencialmente fatais.
